You'd better to go to the local police station public counter and tell them your current address and contact phone number. The Police officer will change your details in their system. Because the Court will later send you summons. If police can't find you to serve the court summons, they have to apply a warrant for your arrest.
The Traffic Offence Notice is not a form, it is just a notice informing you that you are to be charged with Drunk Driving. The Court date will be in the later court summons that is very important to you. You don't need to worry about the incorrect address but if you want to change, find out the police officer's QID which is on the right bottom of your notice and then contact this officer in charge through the police station. He/She will send you new one if it is necessary.
Please do not drink and drive, you might get killed or kill somebody on the road. It would be too late at that time.
