According to our records,you have either recently received hospital or clinical care,or are on a wait list for treatment from the Auckland District Health Board(ADHB).The right to publicly funded health care is one of benefits that New Zealanders enjoy.In order to keep providing quality care,the ADhb must work to ensure that the patients we treat are in face eligible to receive our services.
Until recently,along with other New Zealand hospital,the ADHB was annually spending millions of dollars on healthcare for people who did not actually qualify for free healthcare and who does not.
We are legally obliged to follow the rules that the government have set.In order to abide by these rules,the ADHB are now asking for proof of identity from all patients.Only those people who meet the eligibility criteria can receive free or subsidized health and disability services.
We understand that this may be an inconvenience,but we respectfully ask for your cooperation as we update our patient records.We will ask for this proof only once,which means that you will not have to show documentation in the future.

Read the attached list of eligibility critera,land determine which category(if any) applies to you
Take photocopies of all the documents requested.
Send these photocopies to us by post,e-mail or fax contact details are listed at the top of this page.
Include your NHI number and this letter with your documents,to make processing quicker.
Please send us this information within 10 days,otherwise if you have received an appointment you will be invoiced.
Regretfully,you will most likely have to pay for your healthcare if you do not meet any of the eligibility criteria or if you can not provide proof of your eligibility.

New Zealand passport
Birth certificate + photo ID
New Zealand
Niue
Tokelau
Cook Islands
Certain groups of people are also eligible,including:
Students
If you have been granted a current work permit in addition to a student permit,which enables you to work in New Zeland for a period of,and which allows for a continuous stay of at least 2 years,you must provide:
All student visas/permits
All work visas/permits
UK Nationals
Australian citizens and residents of Australia
Children
Patients requiring maternity care
If you require maternity care and are not eligible in your own right,you eligible for maternity service if you are the partner or spouse of a New Zealand citizen,or resident,or 2-year work permit holder.
You must provide:
Permits/visas(general and work)
NZ passport
Marriage certificate or proof of prelationship
Refugees:
If you are a refugee you must provide:
A certfincate of Identity
Resident’s visa permint
Asylum seekers:
If you are an asylum seeker you must provide:
Passort of identity documents
Work or visitor permits
A letter from Immigration(received within the last 6 months)confirming you are seeking asylum
