Police certificates
Every person named on the application, and aged 17 years or older, will need to provide:
• a police certificate from their country of citizenship (unless they can provide satisfactory evidence
they have never lived there); and
• police certificates from any country they have lived in for 12 months or more (whether in one visit
or intermittently) in the last 10 years.

You (and any other applicant included in your application) are required to provide a completed General
Medical Certificate (INZ 1007) and a Chest X-ray Certificate (INZ 1096), unless you have previously
provided these certificates and they were issued less than 36 months ago.
