求助以下父母团聚移民补充材料翻译的TEMPLATE:

1.Original Hukouben of the applicants with English translation in the contents
申请人户口本原件及专业英文翻译件

2.Original reference letter with English translation, issued by local residency committee or public security bureau, listing the total number children of theapplicants with details of their names and dates of birth Or original one Child Certificate with English translation if any.
当地居委会或派出所出具的写有申请人的子女总数及子女姓名和出生日期的证明原件和专业英文翻译件-或独生子女证原件及其翻译件或公证书

自己翻译就可以吗？ 还是有一定的形式？

不可以，必须是专门的翻译公司，或者直接去公证处公证都带翻译的
