Who needs to apply for a card?The SuperGold Card will automatically be sent to New Zealand residents who receive New Zealand Superannuation or a Veteran's Pension, or who are aged 65 years and over and receive some other form of assistance from the Ministry of Social Development.
Your New Zealand Superannuation will need to be approved at least three weeks prior to turning age 65 if you wish to receive your SuperGold Card by age 65. If you do not currently receive any of the above listed assistance, you will need to apply for a card.
If you are in any doubt over your eligibility for a SuperGold Card, contact the SuperGold Card Centre on 0800 25 45 65 or send us an email.
How do you apply for a card?To apply for a SuperGold Card, you can:
download the SuperGold Card Application Form and the SuperGold Card brochure (see below)
ask for a SuperGold Card Application Form at your nearest Work and Income Service Centre
contact the SuperGold Card Centre on 0800 25 45 65 to request an Application Form be emailed or posted to you
email the SuperGold Card Centre and request a copy of the Application Form be sent to you.

SuperGold Card Application Form (PDF, 636.91KB) What proof of identity is needed when you apply for a Card?All new applications for a SuperGold Card require two forms of identification.
When applying for a SuperGold Card you will need to provide the following identification from the table below:
one document from column A and
one document from column B (if photo identification is provided) or
two documents from column B (if no photo identification is provided)
One of these documents must show that you have been known by this identity for at least two years (eg driver licence that was issued more than two years ago or a letter from a prominent community member such as a local doctor stating they have known you as “John Smith” for at least the last two years).

Photo page of current New Zealand Passport
Photo page of passport from another country (along with the pages showing your current residency status in New Zealand)
Full New Zealand Birth Certificate
Certificate of Date of Birth (must have been issued prior to September 1995)
Whakapapa (for clients not registered at birth or who cannot be located on the birth register)
Certificate of Registration, Naturalisation, Citizenship or Refugee Status
Confirmation of Permanent Residence, letter from the New Zealand Immigration Service
Certificate of Identity
Application for Refugee Status
For Australian born clients; full Australian Birth Certificate, Passport, or Proof of Residence
For Cook Island, Tokelauan and Niuean clients; full Birth Certificate, Letter of Confirmation, Certificate of Registration, or Naturalisation.
Government organisations other than Work and Income eg firearms licence, photo driver licence, Inland Revenue
An expired New Zealand passport provided it has an expiry date that is no more than 5 years ago
An overseas Birth Certificate
A supplier of goods or services eg a telephone or power account
An employer, past or present eg a letter or pay slip from employer
Bank or insurance company eg a record of bank account transactions
Community, health or education organisation eg school leaving certificate, HANZ 18+ card
Prominent community member; eg reference from Justice of the Peace, doctor, kaumatua.
Where do you send your completed SuperGold Card Application Form?Please take your completed application form and any other required documents to your nearest Work and Income Service Centre or send your application to: The Card Centre, PO Box 5054, Wellington.
Any copies of documents need to be certified. This means that the original document is copied and stamped and shows the full name and signature of the person who sighted them. This person can be a Work and Income staff member, Justice of the Peace, Solicitor or Court Registrar.
How do you get your card by age 65?Please ensure your application, and all supporting documentation, is sent to us at least three weeks prior to your turning age 65. |
